NOTICE OF CHANGE OF MANAGERS

INSTRUCTIONS
Format:
Documents required sent to the Registrar pursuant to the
Act must conform with regulations 6 to 10 of the
Regulations under the Act.
Item 1:
Set out the full legal name of the society and, except
where a number has not yet been assigned, state the
society number.
Items 2,3,4:
With respect to each manager:
(a) Set out first given name, initial and family name;
(b) State full residential address; and
(c) Specify occupation clearly, e.g director, farmer,
geologist.
Signature:
A manager or an authorized officer of the society shall
sign a notice. Upon organization, a signatory to the
Articles shall sign the notice.
The completed document, in duplicate, and the prescribed
fee are to be deposited at the office of the Registrar.
This form is to be signed by a manager or by any two
members authorized by the other members in accordance
with paragraph (a) of subsection (2) of section 61 of
the Act, if this is a Notice of Change of Managers.
